Some help debugging non-working workflows please

Does anyone know if there's a document that describes the workflow mechanism in eZ 3.2-5?

I'm still having dreadful problems with it, but I've noticed that the table ezworkflow_assign is empty, and I wonder if this is relevant. I can see my workflows accumulating in ezworkflow_process and runcronjobs.php returns:

Workflow event deferred to cron job(4): 3

0 out of 3 processes was finished

But the notification is just not happening, hence no approval can occur, so the workflow events remain in place.

So far I can only get workflows/triggers to operate if the editor is set to Admin and the users without approval is set to All. If I change the editor to another user, with all else being the same, then that breaks the workflow mechanism - the user assigned the editorial role never gets to see any workflow notifications.
