create form

I am new to eZpublish....i wanted to create the custom form called contact. i have created the class called contact. how do create the form for contact detail. i wanted to add this form in contact.tpl. when i click the contact in menu i should get contact tempelate and content has form...how do i do this

I have a template called contact.tpl. in this template first i have mentioned few user names and there email id's. after that i wanted the user to fill the form. I have already created the class called contact which contains the fields of contact. now how do i create the form based on this class. please help this is urgent.....
