How to Create a Form

I think the major sticking point for me is that the documentation that described this process resided in the now defunct SDK and so I have no background info on which to base my forms development. Certainly, info on the Information Collector is not be found anywhere!! Has anyone got a step-by-step guide on how to achieve this in 3.2?? Thanks, Don.
