Random "Cannot find server" or 404 errors

I'm new to ezp but for the most part I've successfully installed the system and have a new template working. However, about 30% of the time, the browser fails to load the requested page. This happens on the site and in the admin, though the smytoms are slightly different. In the site the page URL is correct and a simple refresh brings the requested page; whereas in the admin usually the problem is that the url ends in '//' i.e. either there is an extra slash or a missing uri element, but removing the last slash locates the requested page. I've tried various settings in php and Apache but nothing seems to make a difference.
