how to create a custim form page?

Well I tried to add a from in an article mean 1st i created an article and then in its content i wrote form and its fields code and it did not worked, It said unsuspacted tag input has been removed. What if i don't create artcile and go for some other thing, will it support creating form? My main problem is this : that i want to add a file field at : http://www.rigibore.co.uk/index.php/about_us/qotation see this page, it has contact info but there is no file field, i want to add a file field so that users can upload documents etc. thanks Dizyn http://www.dizyn.com

You can enable permissions for anonymous publishing, to allow users to add the files, but that isn't ideal either for a quote page I don't think.
