Problem with required file attribute

When user saves newly created object with uploaded file no error is reported. After that sometimes (with no specific pattern) uploaded file doesn't exist any more. Just realized that every attribute after that file attribute isn't saved either.

How is possible that some attributes are saved and some are not? Isn't the save action in transaction?
