I've got the Form Builder extension up and running on my eZ publish system (Forms tab visible in the administration interface). How does one go about actually using it, i.e., how should one proceed (say, for example, to set up an online survey consisting of a question and three optional answers "yes", "no", and "don't know" that a user would select from using a checkbox) once you've hit the "Add new form" button?
