Empty To: field in collaboration notification e-mails

I'm using the standard eZ Publish collaboration workflow on eZ Publish 4.0.

The approval notification e-mail is correctly sent out, but the "To:" field in the e-mail body is missing! This causes problems as the e-mail is often interpreted as spam.

Has anyone else seen this problem?

Has anyone found a way around it? Do I have to modify the kernel? Or can I get the information I need (recipient's name and e-mail address) in the template file that builds the e-mail (approve.tpl)?
