Weird problem with url aliases

I have a serious problem with url aliases on my site running eZP 3.10.1. Some weird things happen all the time, here are some examples

1) for some nodes urls generated with {$node.url_alias} are in form of "content/view/full/7", and not in form of "news/mynews"

2) I had a node called "Multimedia", and then I created a new node with the same name which got the url alias "Multimedia2". That's OK so far. Then I deleted (completely, not only moved to trash) the first "Multimedia" node, changed the name of node "Multimedia2" to "MultimediaX", and so far everything was still OK. But then I changed the name of "MultimediaX" to "Multimedia". After that this node got the url alias "Multimedia2". And that's the problem, it should have got url alias "Multimedia" since the first multimedia node is now gone.

Some details about my site:

- it was upgraded from eZP 3.9 to 3.10.1
- it uses UTF8 as encoding
- it has TransformationGroup=urlalias

Did anyone have similar problems? Any idea how to find out what's going on?
