capturing more information on a product page

In my product page I have added an information collector field (text box) and a checkbox field.

My question is twofold:

1. How do I force a user to fill in specific fields before the form gets processed?

2. How do I get the data captured from the text field to show as part of the order in the webshop admin page?

would appreciate any pointers on this

I realise that this was some time ago now but did you get anywhere with this?

I too wish to achieve this solution. Can anybody help?
