Odd problem accessing index.php with filematch "."

I've got this commented out on a client site, because when it's enabled the directory index line doesn't work. I get a permission denied error if I try to access the site as / instead of /index.php.

What does this line do, and why is it causing this error? It seens to be working perfectly without the line, so I'm thinking that I'll just leave it out, but I'm not clear on it's purpose, so I'm a tad nervous about doing so.
