As far as I know eZ it's not entirely possible. ezurl always puts index.php in links. One sollution is to use the ezroot like described in the doc: http://ez.no/doc/ez_publish/technical_manual/3_6/reference/template_operators/urls/ezroot but this operator also ommits the siteacces name, so it will work only for the default siteaccess. A suggestion to the eZ team: Could you modify the ezroot operator so that it can take an agument wether to include the siteacces name in the output or not. It would be really neat, and all the people using eZ could have all their sites with nice links without index.php in it (provided that they set the appropriate apache rewrite settings).
