How to display image upload button in Registration form?

I have given image attribute in the user class and user account Attribute also.. But In my registration form it does n't show the image upload option and also it hide the password and confirm password text boxes also.. What mistake has happened.. Or What I have to do to display the password as well as upload image options Please Help. A. Thirunavukkarasu
